To add a witness statement to an incident. 

Background

When incidents occur, collecting statements from witnesses is common practice. A witness statement is taken as part of the incident investigation process and is used to better understand what happened, why it happened, and how similar incidents can be prevented in the future.

Things to Consider

  • Required User Permissions:
    • To add a witness statement: 'Standard' permissions or higher on the project’s Incidents tool.
    • To add a reference user in the 'Witness Name' field (see What is a 'reference user' and which project tools support the concept?):
      • 'Standard' permissions or higher on the project’s Incidents tool
      • 'Read Only' permissions or higher on the project's Directory tool WITH 'Create Reference Users (Only Applicable if Timesheets or Incidents is Enabled)' granular permission enabled on your permission template.
        Notes:
        • Users who have been granted the 'Create Reference Users (Only Applicable if Timesheets or Incidents is Enabled)' granular permission can:
          • Add a new reference user when the person does NOT already exist in the Company or Project Directory.
          • View existing reference users as a drop-down list selection for all projects in the company's Procore account. 
        • Users who have NOT been granted the 'Create Reference Users (Only Applicable if Timesheets or Incidents is Enabled)' privilege do NOT have sufficient permission to add new reference users. However, they do have sufficient permission to view reference users added by others on the project and to select an existing reference user when adding a witness statement.

Prerequisites

Steps

  1. Navigate to the project's Incidents tool. 
  2. Choose from these options:
    • To create a new incident, see Create an Incident
      OR
    • To edit an existing incident, locate the incident and click Edit
  3. Scroll to Witness Statements. 
  4. Click Add Witness Statement.
  5. Complete these steps:
    1. Name. Select a witness's name from the list. 
      Note: If the person's name does NOT exist in the Project Directory, type the person's name and click Add to Project. Then enter the witness's name. 
    2. Date Received. Select the date of the witness statement from the drop-down calendar. 
    3. Statement. Type the witness statement or a comment in this box. 
    4. Attachments. Click Attach Files or use a drag-and-drop-operation to upload a file with the statement or any supplemental information related to the statement. 
  6. Click Create.
