Create a Near Miss Record Within an Incident

Objective

To create a near miss record within an incident.

Things to Consider

  • Required User Permissions: 'Standard' or 'Admin' permissions on the Incidents tool.

Prerequisites

  1. Create an Incident

Steps

Navigate to the Incidents tool, and click Edit next to the incident under which you want to create a Near Miss record. Under Incident Records, click Add Record.

  1. Click Near Miss

  2. Enter the following information, or select "(None)," for applicable drop-down menus.

    • Affected Company: Select the company of the person who was involved from the near miss. This field pulls from users in your project's Directory.

    • Person Affected: Select the person who was involved in the near miss. This field pulls from users in your project's Directory.

    • Work activity: Select the activity involved the person was doing when the near miss occurred.

    • Equipment: Select the equipment involved in the near miss.
      Note: this list is pulled from the Project level equipment list. See Add Equipment.

    • Source of Harm: Select the source of harm that could have affected the person (e.g. material, electrical, chemical).

  3. Click Submit.

  4. Repeat adding records as needed.
