STANDARD OPERATING PROCEDURE (SOP)

Job Hazard Analysis (JHA) – When It Must Be Completed

1. PURPOSE

To establish clear requirements for when a Job Hazard Analysis (JHA) must be completed to identify, evaluate, and control hazards before work begins, ensuring all personnel return home safely.

2. SCOPE

This SOP applies to:

  • All Koester Construction employees (Self Performers)
  • All subcontractors and trade partners
  • All jobsite activities, operations, and tasks

3. POLICY STATEMENT

A Job Hazard Analysis (JHA) is recommended for any task where hazards are present or may develop and mandatory for items in 5.4

Safety is more important than money or schedule. No work shall proceed without proper hazard identification and control

4. RESPONSIBILITIES

Superintendents

  • Ensure JHAs are completed, reviewed, and enforced daily
  • Stop work if JHA requirements are not met

Subcontractors / Foremen

  • Complete and communicate JHAs with crews
  • Update JHAs as conditions change

All Workers

  • Participate in JHA discussions
  • Follow identified controls
  • Stop work if hazards are not addressed

5. WHEN A JHA IS REQUIRED

A JHA is required under the following conditions:

5.1 Before Starting Any New Task

  • First time performing a task on-site
  • New phase of work
  • New crew performing the work

5.2 When Work Conditions Change

  • Weather changes (rain, wind, heat, ice)
  • Site conditions change (mud, access, congestion)
  • Change in work area or environment

5.3 When Introducing New Hazards

  • New equipment, tools, or materials
  • New processes or methods
  • Working near energized systems or utilities

5.4 High-Risk Activities (ALWAYS REQUIRED)

JHAs are MANDATORY WITHOUT EXCEPTION for:

  • Fall protection work (heights, leading edges, scaffolds)
  • Excavation and trenching
  • Crane lifts and rigging operations
  • Confined space entry
  • Electrical work (energized or near energized systems)
  • Hot work (welding, cutting, grinding)
  • Demolition activities

5.5 After an Incident or Near Miss

  • Immediately before resuming work
  • Must include revised hazard controls

5.6 When Required by Management or Safety Team

  • Directed by Job Superintendent, Company Safety Director, and/or OSHA directive

6. JHA REQUIREMENTS

Each JHA must:

  • Break the task into steps
  • Identify hazards for each step
  • Define controls (engineering, administrative, PPE)
  • Be reviewed with the entire crew
  • Be understood by all workers (language appropriate)
  • Be signed/acknowledged by crew members

7. STOP WORK AUTHORITY

Work must be stopped immediately if:

  • A JHA is not completed when required
  • Conditions differ from the JHA
  • Hazards are not controlled

8. DOCUMENTATION & RETENTION

  • JHAs must be available on-site at all times for the duration of the contracted work and stored where the work is being performed
  • Stored in Procore >Job > Photos > Album >JHA folder

9. ENFORCEMENT

Failure to comply may result in:

  • Removal from the jobsite
  • Disciplinary action
  • Contract termination (for subcontractors)
