Add an Injury Record to an Incident

Objective

To create an injury record within an incident.

Background

When you create an incident in Procore, you can add one or more incident records to capture details. There are two types of records that you can add to an incident in Procore: Near Miss or Injury

Things to Consider

  • Required User Permissions:
    • 'Standard' or 'Admin' level permissions on the project's Incidents tool.
  • Prerequisites:
  • Additional Information:
    • Custom options can be created for certain fields in an injury record. See Add Custom Options for Incident Fields.
      Note: These field options for incidents are managed in the Company level Admin tool and can only by users with 'Admin' permissions on the Company Admin tool.

Steps

  1. Navigate to the project's Incidents tool.
  2. Under the Incidents tab, complete the following:
    1. Locate the incident in the list. Then click Edit.
    2. Scroll to Incident Records.
    3. Click + Add Record
    4. Click Injury.
  3. Select the following under Activity Information:
    1. Affected Company. Select the company of the person who was involved from the incident. If you have selected a contact in the Person Affected field, this will automatically populate with the person affected's company name as it appears in your Directory tool.
    2. Person Affected. Select the person who was involved in the incident. If you have selected an affected company, choose from contacts in your Directory who are associated to that company.
      Note: See Update a Person's Company Information.
  4. Select the following under Activity Information:
    1. Work Activity. Select the activity the person was doing when the incident occurred.
    2. Equipment. Select the equipment involved in the incident.
      Note: This list is pulled from the Project level equipment list. See Add Equipment if the equipment you are looking for is not present in this list.
    3. Source of Harm. Select the source of harm to the person (e.g., material, electrical, chemical).
  5. Specify the following under Injury Information:
    1. Injury/Illness. Select the injury/illness that the person sustained from the incident.
    2. Body Part Affected. Select the body part affected by the injury/illness sustained in the incident.
    3. Work Days Absent. Enter the number of work days the person was absent from work because of injuries or illnesses sustained in the incident.
    4. Work Days Restricted. Enter the number of work days the person was restricted from work because of injuries or illnesses sustained in the incident.
    5. Work Days Transferred. Enter the number of work days the person was transferred to another role on the jobsite because of injuries or illnesses sustained in the incident.
    6. Date of Death. Enter the date that the person died because of the incident. 
  6. Specify the following under Treatment Information:
    1. Filing Type. Select the filing type the person chose to use.
    2. Treatment Provider. Enter the name of the treatment provider who treated the person's injuries or illnesses.
    3. Treatment Facility. Enter the name of the facility that the person was sent to for treatment.
    4. Treatment Facility Address. Enter the address of the facility that the person who was sent to for treatment.
    5. Treated in ER. Mark this checkbox if the person was treated in the Emergency Room.
    6. Hospitalized Overnight. Mark this checkbox if the person stayed in the hospital overnight.
  7. Click Submit
